We are seeking a compassionate and dedicated Registered Nurse (RN) to join our team and provide essential nursing care to individuals in the comfort of their homes. As an RN with Advocate Health Care Services, your role will be key in helping clients with their daily living activities, ensuring they receive the care and support needed to maintain their independence and quality of life.
Responsibilities:
- Deliver direct nursing care while adhering to individualized care plans and physicians’ orders.
- Perform comprehensive assessments of client status (e.g., physical, psychosocial, environmental).
- Report significant changes in client status to the physician and other team members.
- Administer medications, injections, and treatments as prescribed.
- Monitor and document vital signs, symptoms, and client responses.
- Assist with personal care tasks such as bathing, dressing, grooming, and toileting as needed.
- Educate clients and their families on health conditions, medications, and care techniques.
- Maintain accurate, timely, and detailed electronic health records (EHR) documentation.
- Communicate effectively with clients, families, physicians, and other healthcare professionals.
- Demonstrate safe practice in the use of equipment.
- Follow infection control and safety procedures to ensure a safe care environment.
- Respond to emergencies and report any significant changes in condition promptly.
- Participate in care plan updates and interdisciplinary team meetings.
- Supervise applicable team members as delegated by the Nursing Supervisor.
- Complete all required training in accordance with agency guidelines.
Qualifications:
- Current and unrestricted Registered Nurse (RN) license in the state of Minnesota.
- Current and valid Basic Life Support (BLS) certification.
- Minimum of one year of nursing experience, preferably in home care or a related field.
- Proficiency with electronic documentation and medical software.
- Strong organizational and time-management skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and adapt to changing client needs.
- Reliable transportation and a valid Minnesota Driver’s License.
- Ability to lift and assist with physical tasks as required.
- Must pass a background check and drug screening.
Hours:
- Overnight shifts from 10 p.m. to 7 a.m.
- Availability on weekends and holidays may be required.
